The support is placed on the fascial or SMAS layer of tissue.

A face lift involves raising of the tissues of the face and resuspending them for an improved skin surface with less wrinkles and folds.  The raising of the tissues is done at three different levels.

Subcutaneous level

Subfascial or sub-SMAS level

Subperiosteal level or at the level of the bone

I prefer the subfascial or intermediate level.  This level is known as the SMAS in Plastic Surgery.  I feel it has the following advantages:

Support with less tension on the skin

Improved blood supply to the skin

Longer lasting support for the lift.

No alteration of the basic facial shape

Smoother appearance of the skin around the eyelids.

This patient has had the lower two thirds of the face lifted.  The forehead and eyebrows were not lifted.  She also has not had the benefit of eyelid surgery.

This woman had the supporting layer lifted at both the area around the eyes and the face and neck.  This total support produces a smooth contour of all three areas.  If the upper face is not lifted with the brow there is often crowding of the tissues around the eyes.
